Medical Form

Planning on camping with the pack? Day Camp? STEM Camp? You'll need a BSA medical form which you can download below. The original will need to be turned in to a designated leader or camp director, but you can keep a copy with you as well, just in case. If you'd like, download the PDF form and keep it on your computer so you can edit it as time going on, then print it out to sign and date as needed.

Public Photo Release Form

We like showing off our scouts, but safety first. When you signed up for cub scouts you agreed to let your scouts be photographed at scouting events, but that covers general large public places that your scout might attend. Below is a release, specifically for our pack, to use your child's image, appropriately. Print it, sign it and bring it to your leader at the next den/pack meeting. There are 2 options to check, 1) PRIVATE Facebook family group where most images are put from pack or den events 2) PUBLIC website or recruiting usage on posters and/or slideshows. You may accept 1, both or no options.
